Virtual Caregiving/Telehealth Program

Keeping clients healthy at home is the goal. With our Virtual Caregiving through Telehealth Program, clients are provided with additional support. Our Virtual Caregiving Services extend beyond the time caregivers can be at your home. We continuously care for your loved one's well-being even when no caregiver is physically present.

How Do We Make This Possible?

  • We assign live 24/7 support by our Remote Care Coordinators who are trained to be our care recipients’ friends and monitor health conditions.
  • We identify falls, wandering, and other safety concerns.
  • We provide timely interventions if anything outside the norm occurs and notify families or emergency call centers.
  • We prevent costly hospital readmissions that may be avoidable through monitored care.
  • We bridge External Care Services by coordinating transport and helping replenish other needed supplies when necessary.
  • We address psychosocial needs through our Remote Care Coordinator who brings in additional support for isolated seniors and creates meaningful connections.

Care America HomeCARE offers companionship as part of our overall care plan. These typically entails making regular visits whose frequency will solely rely on the care recipient’s needs. 

Managing everyday household tasks for older independent adults may eventually be challenging. Companion care is often the first way families seek help to enable their aging loved ones to remain in their own homes. This is especially true when they experience loss of mobility with aging or cognitive decline from the onset of Alzheimer’s disease or dementia. If a loved one prefers to age-in-place, companion care will enable them to remain independent, in their own home. Companionship is a valuable benefit for older adults not just those who face challenges in keeping up with daily household chores, but are at risk of isolation, or need transportation to appointments and other locations. 

For family members who work full-time or have other obligations, it can be difficult to ensure that a member of the family is always available to transport a senior loved one to regular doctor appointments and help with other errands like grocery shopping. These seniors, particularly if they no longer drive and lack access to convenient transportation, can benefit from companion care. Care America HomeCARE is a registered provider for the VA’s Homemaker and Home Health Aide Care Program. This program is for Veterans who are in need of skilled services and help with activities of daily living.

As a VA registered provider, we will work with your VA Social Worker to provide you with our Personal Care services as a part of an alternative to nursing home care, and to get respite care at home for Veterans and their family caregiver.  We can help Veterans remain living in their own home and can serve Veterans of any age.

24-Hour Care

Care America HomeCARE offers prolonged shifts, most commonly in 8, 12 or 24-hour increments, although you may choose any amount of care ranging from a 4-hour mini-visit to 24-hours of care every day.

A 24-hour shift is designed for aging loved ones that want MORE than occasional or limited assistance.

When your aging loved-one needs help at home, you might feel quite overwhelmed and at a loss for what you could do and what your options are.

This can happen when your aging loved-one encounters an unplanned, accidental health challenge. We also find that this can occur when your loved-one has a planned operation (like a knee replacement) that will require a recovery period later.

Hourly Care vs. Live-In Care

24-Hour Care in San Francisco, CA

Live-In Care

Before a client can get a live-in aide, the members of the household are required to supply the caregiver with sleeping accommodations. The family must agree and understand that the aide is eligible to sleep at least eight hours each night.

You will find that sometimes an aging adult might need assistance at nighttime, but regulations require the aide get a minimum of five hours of uninterrupted sleep every night.

A live-in aide can only work in a house for up to four consecutive days per week, then needs to be replaced with a second aide for the remaining days.

Live-in aides are compensated on a daily as opposed to an hourly rate since they are regarded as “off-duty” while they sleep.

Hourly Care

The caregiver (aide) must be alert and accessible at all times. This type of care needs at least 2 home health aides in a 24-hour period-of-time. The shifts can be divided into two 12-hour shifts or three 8-hour shifts.

Aides are NOT expected to sleep during their shift.

A client might need split-shift or hourly care because (s)he must be repositioned in their bed every few hours, need help to the bathroom several times each night or might be confused and try to wander at night.
